The proof of financial support documentation can be in the form of a bank statement (student's or parents') or other document stating the student will have at least $6,600 for the semester. This form must be completed early because MSU must process the J-1 Visa Form in the United States. Once processed in the United States, MSU must send the form to the incoming exchange student in their home country. The student must take the J-1 Visa Form they received from MSU to the US Embassy in their home country (with the original proof of financial support documentation) and apply to receive their J-1 Visa.
TOEFL Requirement
Students coming to Michigan State University from NON-English Speaking Countries must provide MSU with their TOEFL Scores. The MSU Exchange Student Requirements are a total score of 550 and a score of at least 52 on the three subsections of the exam. If students take the Computer Based TOEFL, the MSU Exchange Student Requirements are as follows: Total Score of at least 213, Listening at least 18; Structure/Writing at least 20; Reading at least 19.

This form is used to reserve campus housing for the students. After students sign the housing contract, they will not be permitted to move off-campus without paying a fine of $400 US.
Airport Pickup Form (Only required if the student wants to be met at the Lansing Airport) [download form now (PDF)*]
This form is used to arrange for free airport pickup. Free pickup is avialable only on designated arrival days and at the Capital City Airport in Lansing, Michigan. Students arriving on different days or at different airports will have to arrange their own transportation to MSU.
Student Insurance Waiver Form (Only required if attempting to waive the MSU Insurance)
OSA highly recommends purchasing MSU student insurance. However, students with other policies considered comparable to MSU coverage can apply for a waiver permitting them to opt out of the MSU policy. Students should workdirectly with MSU's Human Resources Office to apply for the waiver. They must activate their MSU Email accounts first.
The Student Insurance Waiver Form is available online.
Then, students must provide a copy of their insurance policy (in English), including a detailed description of coverage. Amounts of coverage must be converted into US dollars. The copy of the policy goes to:

After completing the Student Insurance Waiver Form and once the Benefits Office has received a copy of the policy, the student can check the status of the waiver request.
